폐포단백증 환자에서 전폐세척술의 마취관리 1례

Abstract

Pulmonary alveolar proteinosis is a rare disorder in which an amorphous, insoluble, proteinaceous material, rich in phospholipid, deposit in alveoli and bronchioles. The primary disturbance produced by this disorder is impairment in oxygen transfer to the blood perfusing the involved alveoli. This disease was first described in 1958 by Rosen et al. The cause of this disease is still unknown.

Whole lung lavage is the only successful treatment for this disease. The first case of pulmonary alveolar proteinosis treated by whole lung lavage in Korea was reported in 1989.

We report our experience of the anesthetic management for whole lung lavage in 47year old patient who was diagnosed as pulmonary alveolar proteinosis. At first, whole lung lavage of left lung was carried out succesfully, followed by right lung lavage 1 week later. After treatment, he showed improvement in symptom and the lesion on the chest CT decreased.
