p53 변이를 동반한 방사선 저항성 두경부암 세포주에서 대사약물의 치료 효능 기전 연구

Abstract
p53은 가장 잘 알려진 종양억제 유전자 중의 하나로, 다양한 스트레스 자극에 대해 세포 내 cell cycle, apoptosis 그리고, DNA repair 등에 관련된 유전자들의 발현을 조절한다. 최근에는 세포대사에 있어 p53 wild type이 세포의 당대사를 억제하는 것으로 알려져 있다. 본 연구에서는 두경부암 세포주를 대상으로 장기간 반복적인 방사선을 조사하여 p53 변이에 따른 방사선 저항성 세포의 세포대사 변화와 대사약물의 치료효과를 관찰하고자 한다. 두경부암 세포주 중 p53 wild type의 HN30과 p53 null type의 UMSCC1세포주를 이용하여 2-Gy의 방사선을70-Gy가 되도록 반복적으로 조사한 후 clonogenic assay를 통해 확인된 방사선 저항성 세포주를 HN30-R, UMSCC1-R로 표기하였다. 만들어진 방사선 저항성 획득 세포의 세포대사를 확인한 결과, p53 null type의 방사선 저항성 암세포주 (UMSCC1-R) 에서는 UMSCC1세포보다 glucose uptake와 lactate production이 증가하고 당대사 중간 산물인 3-phosphoglycerate (3-pg)와 pyruvate, 그리고 당대사에 관여하는 효소인 glucose transporter type 3 (GLUT3), hexokinases-II (HKII), lactate dehydrogenase-A (LDHA)가 증가하는 것을 확인하였다. 하지만 p53 wild type인 방사선 저항성 암세포주(HN30-R)는 HN30과 비교 했을 때, 세포대사의 변화가 없었다. In vitro에서 당대사 표적 치료제인 Hexokinase inhibitor (2-Deoxy-D-glucose, 2-DG) 와, lactate dehydrogenase-A inhibitor (AT101) 에 대한 세포 반응성를 확인한 결과, p53 wild type인 HN30과 HN30-R 에서는 세포사멸에 변화가 관찰되지 않은 반면, p53 null type인 UMSCC1-R 에서는 2-DG와 AT101에 세포사멸이 UMSCC1보다 증가하는 것이 관찰되었다. 이는 p53 null type의 in vivo xenograft model에서도 UMSCC1-R에서 2-DG에 대한 종양억제 효과가 증가되는 동일한 결과를 얻었다. 이러한 결과들은 p53 null type (UMSCC1)은 방사선 저항성을 획득하면서 당대사 의존도가 증가하는 것이 관찰되었지만, p53 wild type (HN30)은 방사선 저항성을 획득하여도 당대사에는 변화가 관찰되지 않았다. 이를 통해 향후 방사선 저항성 획득으로 인한 치료 실패 시, p53 변이 확인을 통한 당대사 표적 약물의 병행 치료는 재발암 치료예후 향상에 효과적인 방법이 될 것으로 판단된다.|p53 is one of the best known tumor suppressor genes and regulates the expression of genes involved in intracellular cell cycle, apoptosis and DNA repair in response to a variety of stress stimuli. Recently, p53 wild-type cell metabolism is known to inhibit glucose metabolism in cells. In this study, we examined long-term repeated irradiation of head and neck cancer cell lines to observe the changes of the cell metabolism and the therapeutic effect of the metabolism drug on the p53 mutation. The HN30 cell lines of p53 wild type and UMSCC1 cell lines of the p53 null type in Head and neck cancer cell lines were repeatedly irradiated with 2-Gy to 70-Gy, and the radiation-resistant cell lines identified by the clonogenic assay called each HN30-R and UMSCC1-R. After confirming the cellular metabolism of the radiation-resistant cells, compare to UMSCC1 cells, the p53 null type radiation-resistant cancer cell lines (UMSCC1-R) increased glucose uptake and lactate production, elevated glycolytic intermediates including 3-phosphoglycerate (3-pg), pyruvate, and upregulated glycolytic enzymes including glucose transporter type 3 (GLUT3), hexokinases-II (HKII) and lactate dehydrogenase-A (LDHA). But p53 wild type radiation-resistant cell lines (HN30-R) exhibited similar results to HN30 cells for glucose uptake, lactate production, and extracellular acidification rate as well as glycolytic intermediates and related glycolytic enzymes. We tested the sensitivity of the recurrent cancer cells after radiotherapy to glycolysis-targeting drugs such as a hexokinases inhibitor (2-deoxy-ᴅ-glucose; 2-DG) and a lactate dehydrogenase-A inhibitor (AT101). UMSCC1-R cells exhibited an increased sensitivity to 2-DG and AT101 but HN30-R cells did not show any changes. The same results were obtained in the in vivo xenograft model of p53 null type in which the tumor suppression effect on 2-DG was increased in UMSCC1-R.
These results showed that the p53 null type (UMSCC1) were more dependent on glycolysis while acquiring radiation resistance, but the p53 wild type (HN30) showed no change in glucose metabolism even after obtaining the radiation resistance. Therefore, concurrent treatment of glucose metabolism target drug with confirmation of p53 mutation will be an effective method for improving the prognosis of recurrent cancer in case of treatment failure due to acquisition of radiation resistance in the future.
