ADHD 아동과 일반 아동의 한국 웩슬러 아동지능검사 3판 및 4판 수행에 대한 문헌분석

Abstract
This study aims to conduct comparative analyses of subtest and index performance in the papers that reported K-WISC-Ⅲ and K-WISC-Ⅳ performance in ADHD children and ordinary children to understand the intellectual characteristics of ADHD children and to draw interventions for them. To derive data, this study selected papers that reported the performance of K-WISC-Ⅲ and K-WISC-Ⅳ in ADHD children and ordinary children from scholarly journals and theses/dissertations listed in the Research Information Sharing Service (RISS) between 2000 and 2020. Next, raw data were derived using R 4.0, based on the case number (n), mean (M), and standard deviation (SD) presented in each paper, and t-tests were conducted, using SPSS 25.0 to examine the difference in the performance between the groups. The results were as follows.
An analysis of the aspects of the performance of the K-WISC-III subtests, indices, and full-scale IQ by the groups of ADHD children and ordinary children revealed that the score was the highest in the performance of the similarity subtest (SI) in both ADHD children and ordinary children, but ADHD children had the lowest score in the coding subtest (CD), and ordinary children, in the digit-span subtest. As the performance was highest in verbal IQ and lowest in the processing speed index in both ADHD children and ordinary children, there was a similarity in the aspects of index performance.
A comparative analysis of the differences in the performance of the K-WISC-Ⅲ subtests, indices, and full-scale IQ between ADHD children and ordinary children revealed that ADHD children showed significantly lower performance in all subtests, indices, and full-scale IQ compared to ordinary children. The mean differences in the subtests between the two groups were largest in the coding subtest (2.05) and the understanding subtest (2.05) and smallest in the block design subtest (.75). The mean differences in the indices and the full-scale IQ were largest in the attention index (10.04) and smallest in the full-scale IQ score (6.56).
An analysis of the aspects of the performance of the K-WISC-Ⅳ subtests, indices, and full-scale IQ by the groups of ADHD children and ordinary children revealed that ADHD children had the highest score for block design, and ordinary children, for picture completion in the performance of the subtests while ADHD children had the lowest score for coding, and ordinary children, for digit span. In ADHD children, the performance was highest in perceptual reasoning and lowest in working memory. In contrast, in ordinary children, the performance was highest in verbal comprehension and lowest in processing speed.
A comparative analysis of the differences in the performance of the K-WISC-Ⅳ subtests, indices, and full-scale IQ between the groups of ADHD children and ordinary children revealed that ADHD children had significantly lower performance in the subtests, indices, and full-scale IQ, except for the block design subtest, compared to ordinary children. The mean difference in the subtests between the two groups was largest in picture completion (2.29) and smallest in block design (.52). The mean difference in indices and full-scale IQ was largest in working memory (11.33) and smallest in perceptual reasoning (4.58).
Thus, in K-WISC-Ⅲ performance, ADHD children showed overall lower intellectual performance than ordinary children but showed a similar aspect of the order of index performance. However, ADHD children also showed a lower overall intellectual performance compared to ordinary children in K-WISC-Ⅳ performance but showed a different aspect of the order of index performance from that of ordinary children. There was a different aspect of the order as ADHD children’s K-WISC-Ⅳ index performance was highest in perceptual reasoning, followed by verbal comprehension, processing speed, and working memory, while ordinary children’s K-WISC-Ⅳ index performance was highest in verbal comprehension, followed by perceptual reasoning, working memory, and processing speed. Through the above, it is found that K-WISC-Ⅳ distinguishes ADHD children better than K-WISC-Ⅲ.
The subtests and indices that show the biggest mean difference in the performance of the subtests and indices of the K-WISC-Ⅲ and K-WISC- Ⅳ between the two groups are the coding and comprehension (CO) subtests and the processing speed index in K-WISC-Ⅲ and the picture completion and coding subtests and the working memory index in K-WISC-Ⅳ. The areas of the subtests and indices that had large mean differences between the two groups would be examined in detail as follows.
First, the result that the difference was the largest in the performance of the coding subtest measuring sustained attention can be interpreted that ADHD children have difficulty with sustained attention, concentration, and visual memory. Also, both the comprehension subtest and the picture completion subtest measure social coping ability. The comprehension subtest measures the ability to understand social rules and concepts and solve problems and can understand the levels of the development of conscience and the sense of morality, in other words, social maturity. This result can be interpreted that ADHD children’s flexibility to solve problems by understanding social rules and concepts and the clues of social-adjustment function and the development of conscience and the sense of morality. In addition, the result that the difference was the largest in the performance of the picture completion subtest measuring the ability to differentiate the essence and non-essence of things can be interpreted that ADHD children have difficulty with the ability to understand important and core clues in living environments quickly and concentration.
For indices, the difference was the largest in the performance of the processing speed measuring the ability to process information accurately and quickly and the working memory index measuring the input function, the retrieval of information from long-term memory, the executtion of strategic processes, the process of paying attention and executing, and the ability to classify things stored in the language and space-time. This result means that, in ADHD children, cognitive control has declined, which is the ability that allows them to act or react for the objective of a specific task and act for the demand for the task they currently face by suppressing disruptive behaviors or habitual reaction tendencies.
This study could understand the intellectual characteristics more objectively, centering around the subtest and index performance of relatively more ADHD children through the reviews of the literature on the K-WISC-Ⅲ and K-WISC-Ⅳ performance in ADHD children and ordinary children between 2000 and 2020. In addition, it was found that, for ADHD children, an intervention program would be necessary, which could enhance cognitive control by suppressing distraction and impulsiveness. Through this, it would be possible to carry out education and intervention strategies appropriate for the sites of childhood education, clinical scenes, and parent and teacher education. |본 연구의 목적은 ADHD 아동과 일반 아동의 K-WISC-Ⅲ, K-WISC-Ⅳ 수행을 보고한 논문에서 소검사 및 지표 수행을 비교분석하여 ADHD 아동의 지적 특성을 이해하고 아동을 위한 중재방안을 도출하는 것이다. 자료수집을 위해 학술연구정보서비스(RISS)에서 학술지논문과 학위논문 중 ADHD 아동과 일반 아동을 대상으로 K-WISC-Ⅲ, K-WISC-Ⅳ 수행을 보고한 2000년∼2020년 사이의 논문을 선정하였다. 다음으로 각 논문에 제시된 대상 수(n), 평균(M), 표준편차(SD)를 활용하여 R 4.0 프로그램으로 원자료를 산출하였고 집단 간 수행 차이를 알아보기 위해 SPSS 25.0 프로그램을 사용하여 t 검정을 실시하였다. 본 연구의 결과를 정리하면 다음과 같다.
ADHD 아동과 일반 아동 집단별로 K-WISC-Ⅲ 소검사, 지표 및 전체IQ 수행양상을 분석한 결과, ADHD 아동과 일반 아동은 모두 공통성 소검사에서 가장 높은 수행을 보였지만 ADHD 아동은 기호쓰기 소검사, 일반 아동은 숫자 소검사에서 가장 낮은 수행을 보였다. ADHD 아동과 일반 아동은 모두 언어성IQ에서 가장 높은 수행, 처리속도지표에서 가장 낮은 수행이 나타나 지표수행 양상에서 유사성을 보였다.
ADHD 아동과 일반 아동 간 K-WISC-Ⅲ 소검사, 지표 및 전체IQ 수행차이를 비교분석 한 결과, ADHD 아동은 일반 아동에 비해 소검사, 지표 및 전체IQ 모두에서 유의하게 낮은 수행을 보였다. 두 집단 간 소검사 평균차이는 기호쓰기(2.05)와 이해(2.05) 소검사에서 가장 크게, 토막짜기(.75) 소검사에서 가장 작게 나타났다. 지표 및 전체IQ의 평균차이는 주의집중(10.04)지표에서 가장 크게, 전체IQ(6.56) 점수에서 가장 작게 나타났다.
ADHD 아동과 일반 아동의 집단별로 K-WISC-Ⅳ 소검사, 지표 및 전체IQ 수행양상을 분석한 결과, ADHD 아동은 토막짜기, 일반 아동은 빠진곳찾기 소검사에서 가장 높은 수행을 보였고 ADHD 아동은 기호쓰기, 일반 아동은 숫자 소검사에서 가장 낮은 수행을 보였다. ADHD 아동은 지각추론에서 가장 높은 수행, 작업기억에서 가장 낮은 수행을 보였다. 반면, 일반 아동은 언어이해에서 가장 높은 수행, 처리속도에서 가장 낮은 수행을 보였다.
ADHD 아동과 일반 아동의 집단 간 K-WISC-Ⅳ 소검사, 지표 및 전체IQ 수행차이를 비교분석 한 결과, ADHD 아동은 일반 아동에 비해 토막짜기 소검사를 제외한 소검사, 지표 및 전체IQ에서 유의하게 낮은 수행을 보였다. 두 집단 간 소검사의 평균차이는 빠진곳찾기(2.29)에서 가장 크게, 토막짜기(.52)에서 가장 작게 나타났다. 지표 및 전체IQ의 평균차이는 작업기억(11.33)에서 가장 크게, 지각추론(4.58)에서 가장 작게 나타났다.
따라서 K-WISC-Ⅲ에서 ADHD 아동은 일반 아동에 비해 전반적으로 낮은 지적 수행을 보였으나 지표 수행의 순위에서는 유사한 순서양상을 보였다. 반면 K-WISC-Ⅳ 에서도 ADHD 아동은 일반 아동에 비해 전반적으로 낮은 지적 수행을 보였으나 지표 수행의 순위에서는 일반 아동과 다른 순서양상을 보였다. ADHD 아동의 K-WISC-Ⅳ 지표 수행 순서는 지각추론, 언어이해, 처리속도, 작업기억 순으로 나타났으나 일반 아동의 K-WISC-Ⅳ 지표 수행 순서는 언어이해, 지각추론, 작업기억, 처리속도 순으로 나타나 순서양상에서 차이를 보였다. 이것을 통해 K-WISC-Ⅲ 보다 K-WISC-Ⅳ 가 ADHD 아동을 보다 잘 변별하고 있음을 알 수 있다.
두 집단 간 K-WISC-Ⅲ 및 K-WISC-Ⅳ 수행에서 가장 큰 평균차이를 나타내는 소검사와 지표는 3판에서 기호쓰기, 이해 소검사와 처리속도 지표이고 4판에서 빠진곳찾기, 기호쓰기 소검사와 작업기억 지표이다. 두 집단 간 평균차이가 크게 나타난 소검사 및 지표 영역을 세부적으로 살펴보고자 한다.
먼저 지속적 주의력을 측정하는 기호쓰기 소검사 수행차이가 가장 크게 나온 결과를 통해 ADHD 아동이 지속적인 주의력, 집중력, 시각적 기억력 등에 어려움이 있는 것으로 해석할 수 있다. 또한 이해, 빠진곳찾기 소검사는 모두 사회적 대처능력을 측정한다. 이해 소검사는 일상생활에서의 사회적 규칙 및 개념에 대한 이해 능력과 문제해결 능력을 측정하며 양심과 도덕의 발달 정도, 즉 사회성숙도를 파악할 수 있다. 이처럼 ADHD 아동이 사회적 규칙 및 개념, 사회-적응적인 기능에 대한 단서를 파악하여 문제를 해결하는 유연성과 양심과 도덕적 감각의 발달이 저하되어 있음을 해석할 수 있다. 또한 사물의 본질과 비본질을 구분하는 능력을 측정하는 빠진곳찾기 소검사 수행차이가 크게 나온 결과를 통해 ADHD 아동이 생활환경에서 중요하고 핵심적인 단서들을 민첩하게 파악하는 능력, 집중력 등에 어려움이 있는 것으로 해석할 수 있다.
지표에서는 정확하고 빠르게 처리하는 능력을 측정하는 처리속도지표와 입력기능, 장기기억으로부터 정보를 인출하기, 전략적 과정을 실행, 주의를 기울이고 실행하는 과정, 언어와 시공간에 저장된 것을 구분하는 능력을 측정하는 작업기억지표에서 수행차이가 가장 크게 나타났다. 이와같은 결과는 ADHD 아동이 특정한 과제 목적에 부합하는 행동이나 반응을 하도록 하고 방해되는 행동이나 습관적 반응 경향성을 억제 시킴으로써 현재 당면하고 있는 과제 요구에 맞게 행동하게 하는 능력인 인지적 통제력이 저하되어 있음을 의미한다.
본 연구에서는 2000년에서 2020년사이 ADHD 아동과 일반 아동의 K-WISC-Ⅲ 및 K-WISC-Ⅳ 수행에 대한 문헌조사를 통해 비교적 많은 수의 ADHD 아동의 소검사 및 지표 수행을 중심으로 보다 객관적으로 지적 특성을 이해할 수 있었다. 또한ADHD 아동에게는 산만함, 충동성을 억제하여 인지적 통제력을 향상시키고 사회적 대처능력을 발달시킬 수 있는 중재 프로그램이 필요함을 확인하였다. 이를 통해 아동교육 현장, 임상장면 및 부모와 교사교육을 위해 적합한 교육 및 중재 전략을 펼칠 수 있을 것이다.
